While I was admiring the underside of my bran new 19 Silverado I noticed the exhaust pipe, immediately after the cat, is squished super flat... Has anyone replaced that section with a normal round piece of tubing? Did it make any improvements in sound/mpg? Seems like it must!

Yes, that is normal.  It wouldn't make any difference if you replaced it with a round tube.  There are threads on the Camaro forum where they have the same thing and did tests with it and with round and it didn't make a difference.

Is entirely correct.

It will not make a difference switching it out. Now if you want to service you trans a lot more often then the book states, switching it out with a straight pipe and  V-band clamps so its easier to remove the exhaust, this would be the best.
