Backfiring through the Intake

I have a 2000 GMC Sierra 1500 4.3L and when starting it backfires through the intake. Not every time but its always difficult and sometimes incredibly rough to start. It still starts but i have to hold the accelerator pedal down and usually takes a few tries. Once its running, it runs perfectly fine. Confused.
